INTRODUCTION –Family Medical Point

We are a not-for-profit organization that advocates, promotes and provides rights-based comprehensive sexual and reproductive health services to young people, women, marginalized hard-to-reach and forgotten communities. 

We enhance access to life saving healthcare services, particularly comprehensive SRH services among minorities, key populations, marginalized communities and young people. We correspondingly promote health in schools and empower women and girls to realize their full potential.

Established in 2017 to respond to direct needs of peri-urban, rural and lakeshore based fishing communities, FMP delivers healthcare services, community health education and promotion, advocacy and influencing policy reform and community sensitization and mobilization. 

FMP runs Medical Centers, Mobile Clinics (Health Caravan) and community outreaches in slum areas, fishing communities on the shores of Lake Victoria and other hard-to-reach locations. We deliver comprehensive rights based sexual and reproductive health services to women young people in and out of school. 

SCOPE OF THE WORK 

FMP plans to develop an annual report for the year 2023 to provide a transparent account of an organization’s activities, achievements, and financial performance over the past year. This will provide a platform to showcase the positive impact and outcomes of the organization’s programs and initiatives. 

Purpose/ Objectives; 

To compile and document FMP’s activities, achievements, challenges, and financial performance throughout the year 2023.

To ensure transparency and accountability by presenting a clear and accurate account of FMP’s operations, including the utilization of funds and the impact of programs.

To assess and showcase the impact of FMP’s healthcare services, projects, and advocacy efforts on the targeted communities, especially young people, women, and marginalized groups.

To contribute to FMP’s public image by presenting a professional and well-documented annual report, showcasing the organization’s commitment to its mission and values.

ELEMENTS OF THE ANNUAL REPORT WILL INCLUDE BUT NOT LIMITED TO:  

  1. Executive Summary

Provide an overview of the Annual Report.

  • Introduction; 

Set the stage for the report, introducing FMP and its activities in 2023.

  • Program Highlights:

Detail FMP’s programs, emphasizing healthcare services, and project activities.

  • Financial Overview:

Present a breakdown of FMP’s budget, expenditures, and funding sources.

  • Challenges and Solutions:

Address challenges encountered by FMP and strategies employed to overcome them.

  • Impact and Success Stories:

Showcase the positive impact of FMP’s initiatives through stories and testimonials.

  • Future Plans:

Outline FMP’s strategic goals and plans for the future.

DELIVERABLE OUTPUTS 

The consultant shall deliver a well-structured and visually appealing Annual Report in both print and digital formats.

The consultant will Provide all raw data, interview transcripts, and supporting documents used in the report.

Develop and submit a draft report with action pictures. Ensure that there are some photographs that can be included in the report. Observance of consent and media protocols. 

METHOD 

The consultant shall obtain data, review relevant literature, interviews, and undertake meetings with selected participants. The consultant is permitted to form a team for the delivery of the objectives of the consultancy. 

APPLICATION MATERIALS: WHAT TO SUBMIT? 

Interested parties should submit a brief proposal, budget and work plan for how the work will be carried out. An attachment of a sample of the previous experience/work. 

DURATION OF THE ASSIGNMENT. 

This work is supposed to be carried out for a period of three weeks from the time of contract signing. The work duration is 8th to 26th January 2024 and the consultant should clearly mention any restriction to start on the planned date and constraints. Any time extensions will be done in consultations with FMP and no additional cost will be made to that effect. 

REMUNERATION AND METHOD OF PAYMENT.    

  • The Consultant shall receive 40% of the final cost after delivery and acceptance of the work plan and signing the contract and 60% of the final cost after the delivery and acceptance of a satisfactory report based on the terms of reference plus meeting all deliverable outputs  
  • The payment shall be by cheque or bank transfer 
  • Penalty: The consultant is subject to a 15% reduction of the total agreed once the report is submitted two weeks after the agreed timelines (without just and reasonable mutually agreed explanation)  

Contract Management 

The consultant will work with the direct supervision of the Team Lead FMP Programs Manager and programs coordinator.
